A fee of $8.00 is required once you submit the request for your nametag.

You must also resubmit a request and pay the $8.00 fee if nametag is lost or broken.

*No order will be submitted until the payment is received. The Department of Curriculum and Instruction has a policy that you must wear your nametag at the field based blocks and during student teaching.
