A fee of $8.50 is required once you submit the request for your nametag.

To complete the order, please mail or drop off your check or cash to the following address:

You must also resubmit a request and pay the $8.50 fee if nametag is lost or broken.

*No order will be submitted until the payment is received. The Department of Curriculum and Instruction has a policy that you must wear your nametag at the field based blocks and during student teaching.
