This form is only for courses taught by the C&I Department. Even if you are an Education major, you will need to contact the correct department that is teaching that course (i.e. requests for Math, PE, Physics, etc. all need to go their dept.). Education courses in the C&I Department include those with the prefix CI, ECE, EDTC, RDG, or SPED.

You will need to fill this form out if courses are set for special departmental approval, with the exception of field-based block courses that require a separate application.

If you have transferred/substituted credits or courses in progress for some of the prerequisites, the system may not fully recognize them as completed (including hours). You will need to fill out this form.

CLOSED COURSES: If a closed course's seating is full to its classroom capacity, an appeal/override cannot be approved due to fire safety regulations. Safety Regulations precede any professor unofficial approvals. Please keep watching the seating on CatsWeb for openings.
